The goal of Hope & Esperanza Community Health Center is to provide culturally competent, affordable health care for all members of the community. Everyone is welcome, regardless of the ability to pay.

Come to the practice for general medical services like treating allergies and asthma care, COVID-19 testing, and management of chronic conditions, like diabetes and hypertension. You can also take advantage of women’s health care and STD testing. If you’re struggling with mental health concerns or substance abuse, the providers are ready to help.

Meet Our Providers

Clemaine C. Mitchell, DNP, BHS, APRN, FNP-BC

Dr. Clemaine C. Mitchell is a dedicated Family Nurse Practitioner with over 20 years of nursing experience, holding a Doctor of Nursing Practice (DNP) from Chamberlain College of Nursing. She is an Advanced Practice Nurse licensed in both New York and New Jersey. Dr. Mitchell has extensive experience in acute and urgent care settings, including roles at University Hospital in Trauma Critical Care and RWJBH Somerset Hospital as an APN in Acute Care and as a Hospitalist. Her skills include suturing, stapling, I&D, EMR, ABGs collection, and central and arterial line placement. She is certified in DOT, ACLS, BLS, and PALS.

Thomas Alaeto, NP

Thomas Alaeto is a Nurse Practitioner with experience in clinical assessment, diagnosis, and treatment of acute and chronic conditions. He holds a Bachelor of Science in Nursing from Seton Hall University and is a Registered Nurse. Thomas has held leadership roles such as ADON/QA Coordinator and Nurse Supervisor, demonstrating skills in quality assurance and staff management. He is also a Certified Dementia Practitioner and QAPI Certified Professional. Thomas is fluent in English and Ibo, with basic conversational Spanish.
